It follows the story of a Seoul-based veterinarian who temporarily moves to a small town called Huidong due to his grandfather's request.
Since the town is far different from where he grew up, he struggled to adapt to his new environment. Thankfully, the local female police officer, Ahn Ja Young warmly welcomed him.
Officer Ahn is played by Red Velvet's Joy while the city-based doctor Han Ji Yul, is portrayed by Choo Young Woo.

'Once Upon a Small Town' Episode 6 Recap: Han Ji Yul's Dark Past
The sixth episode began with Han Ji Yul struggling to save Ahn Ja Young and another police officer after the duo got involved in a car accident.
As Han Ji Yul tries to break the window, he recalls how his family encountered an accident.
In "Once Upon a Small Town" episode 6, a flashback featuring Han Ji Yul and his parents encountered the same accident after their car crashed into the barrier and fell near a creek.
Thankfully, a local farmer came to help them as he broke the glass window at the back seat to save the young Han Ji Yul.
Unfortunately, both of his parents weren't able to survive.
In the next scene, Han Ji Yul stood beside Ahn Ja Young, who is still unconscious at the hospital. At this point, the female officer's health condition is already stable but needs some rest.
Shortly after, Lee Sang Hyun came to visit and thanked Han Ji Yul for saving Ah Ja Young.
Han Ji Yul Receives Heartbreaking Request from an Old Local
Probably one of the touching episodes, "Once Upon a Small Town" episode 6 featured an old local farmer who called the veterinary hospital after his bull was feeling sick for a couple of weeks.
Han Ji Yul visited the farmer to check on his animal. Here, the vet sees that the bull is quite old and shows no hope for his health.
However, the local farmer said that he did not call the hospital for his bull's health but he wanted to have him execute euthanasia or the painless killing of someone who is suffering from an incurable and painful disease.
Initially, Han Ji Yul were surprised to hear this, but he learned that the old man doesn't want his bull to suffer and believes that they will see each other again.
After the devastating decision the old man recalled his first encounter with Han Ji Yul and said that during his parents' death, the old man had to look after the young Han Ji Yul.
Interestingly, the man revealed that Han Ji Yul and Ahn Ja Young had fun childhood memories together during his stay at Huidong.
Here, he finally recalled the little girl who helped him recover during the loss of his parents.
