Oh Soo (Jo In Sung) tried to kiss Oh Young (Song Hye Kyo) several times in the past but every single time, he had stopped himself. The reason for it was simple. He had to stop. Before the moment of their first kiss, Oh Young begins to feel the same about him.

Oh Young begins to touch Oh Soo's face while he is asleep and gets shocked when she feels his lips and takes her hand away. This is when she goes down to the floor to sleep. Oh Young feels something that she didn't even feel when she kissed her fiancé. She doubts that it is anything special since she had met with Jin So Ra earlier the same day.

Anybody can see Oh Young's emotions changing. The only thing left is Oh Young's surgery since Oh Soo's identity has been revealed to everybody. Even though Secretary Wang (Bae Jong Ok) is aware of everything about Oh Soo, she does not reveal it to Oh Young. It is because the only way Oh Young will have her surgery is if Oh Soo makes her.

Oh Young tries to ignore her feelings for her older brother and tries to ignore it. However, it becomes impossible when Oh Soo kisses her lips while she is asleep.

After countless times, he finally kisses her. However, Oh Soo does not look happy, but sad instead. Oh Soo begins to cry. Even though the kiss scene was extremely romantic, there was something very melancholy about it. Regardless of his big move, the kiss is incomplete since he is incapable of showing her everything.

Oh Soo kisses her for the things he can't say to her right now. He is sorry. It is not a confession of his love but it is his way to saying sorry to her. He kisses her because he can't express himself truthfully.

Therefore the meaning behind the kiss is despair. Right now, Oh Soo still has to protect her before he can be a man to her. The kiss seems to express Oh Soo's willingness to die for her, which is why the kiss is incomplete for now.
